Title:Vloga registra ZORA pri obvladovanju bremena raka materničnega vratu v Sloveniji

Abstract:V okviru državnega programa zgodnjega odkrivanja predrakavih sprememb materničnega vratu (DP ZORA) od leta 2003 na Onkološkem inštitutu v Ljubljani deluje tudi koordinacijski center programa s centralnim informacijskim sistemom, imenovanim Register ZORA. V njem se zbirajo vsi izvidi brisov materničnega vratu in histološki izvidi zaradi cervikalne patologije. Poleg tega Register ZORA beleži statistične parametre kakovosti dela in učinkovitosti programa, pripravlja poročila za slovensko in tujo strokovno javnost, deluje kot varnostni mehanizem in vabi na preventivne preglede ženske, ki več kot tri leta nimajo zabeleženega izvida brisa materničnega vratu; odgovoren je za uveljavljanje in izvajanje programa. Končen cilj delovanja DP ZORA je zmanjšati zbolevnost in umrljivost za rakom materničnega vratu. Javnozdravstveni učinki uvedbe programa ZORA v Sloveniji so že vidni. Pregledanost žensk v ciljni populaciji 20 ? 64 let se je dvignila na želeno vrednost 70 %, za ginekologa je opredeljenih več kot 80 % žensk. Podatki Registra raka za Slovenijo kažejo, da se incidenca raka na materničnem vratu v Slovenijo po uvedbi DP ZORA zmanjšuje, hkrati pa se veča incidenca intraepitelijskega raka. Kljub spodbudnim začetnim rezultatom v koordinacijskem centru vidimo še precej možnosti za izpopolnitev delovanja Registra ZORA. V prvi vrsti želimo izboljšati elektronsko povezavo med izvajalci programa in Registrom ZORA ter Registrom ZORA in drugimi državnimi registri, kar precej pa bi na kakovosti pridobili tudi s ponovno vzpostavitvijo pridobivanja podatkov od Zavoda za zdravstveno zavarovanje Slovenije.

Abstract:As a part of national organized cervical cancer screening programme (DP ZORA) a coordination center with central information system named ZORA Registry is operating at the Institute of Oncology since 2003. It registers all diagnoses of cervical smears and all cervical biopsy diagnoses. Further tasks of the ZORA Registry are: inviting women without cervical smear in last three years to take one, monitoring the quality and evaluating the efficiency of all procedures in DP ZORA, organizing regular education for all programme co-workers and providing them a periodic feedback on their efficiency. The ZORA Registry takes care for the promotion of DP ZORA as well. Some of the public health effects of the DP ZORA implementation in Slovenia can already be observed. The smear coverage in the target population (20-64 year) reached the desired value of 70 % and more then 80 % of women have been registered with their personal gynecologist. The data provided by the Cancer Registry of Slovenia show the decreasing trend of the cervical cancer incidence but on thesame time the incidence of the in situ cervical carcinoma is increasing. Despite some incentive initial results there are still many possibilities for improving the ZORA Registry performance. Our first plan is to introduce an online communication among all the DP ZORA co-workers as well as between ZORA Registry and other national registries. Reestablishing of the data exchange with the Health Insurance Institute of Slovenia will help ZORA Registry by inviting the target group more precisely.
