Italian intersocietal recommendations for restructuring the diagnostic-therapeutic pathway for the implementation and appropriate use of anti-amyloid monoclonal antibodies in Alzheimer’s disease

Opis: This joint Position Paper, developed by the Italian Expert Panel on Alzheimer convened by the Italian Society of Neurology with participation from multiple scientific societies, outlines strategic guidelines for reorganizing the patient journey in the era of anti-amyloid monoclonal antibodies for Alzheimer’s disease. Emphasizing a multidisciplinary and integrated approach, the document recommends a patient journey that begins with early identification of cognitive impairment by General Practitioners, continues with specialized assessments at Memory and Dementia Centres, and leads, in carefully selected cases, to initiation of anti-amyloid monoclonal antibody therapy. It advocates the rational use of diagnostic tools, including plasma and cerebrospinal fluid biomarkers, advanced neuroimaging (MRI and PET), and genetic profiling (ApoE genotyping), not only to identify eligible patients but also to stratify those requiring alternative care strategies. The paper further defines minimum requirements for the accreditation of prescribing and infusion centres, highlighting the clinical competencies, structural resources, and inter-professional communication protocols necessary to ensure safety and appropriateness. Recognizing both the therapeutic potential and the organizational challenges associated with anti-amyloid monoclonal antibodies, the document aims to guide healthcare policymakers, institutions, and practitioners toward a coordinated reorganization of the diagnostic-therapeutic pathway, ensuring the safe and effective use of these treatments and ultimately improving outcomes and quality of care for individuals with Alzheimer’s disease.

Unveiling Organizational AI Adoption Patterns in Italian Companies through the Lens of the Diffusion of Innovations Theory

Opis: This paper investigates the adoption and integration of artificial intelligence (AI) technologies within a sample of 237 Italian enterprises using the Diffusion of Innovations (DOI) theory as the theoretical framework. It examines the characteristics of companies leading in AI adoption, evaluating their alignment with the innovator and early adopter profiles defined by Everett Rogers in 2003 within the DOI framework. The research emphasizes AI’s significant role in enhancing operational efficiency, fostering innovation, securing competitive advantage, and driving long-term growth. It also identifies challenges such as lack of skills, data management issues, and ethical concerns. Our findings contribute empirical evidence to the academic literature on the DOI theory, addressing the underexplored context of AI in Italy. The study provides a nuanced perspective on AI’s impact on employment and sets a foundation for future research, offering managerial insights for strategically deploying AI.

Implementation of the Slovenian act on the treatment of children and adolescents with emotional and behavioural problems or disorders in education

Opis: In 2020, Slovenia implemented the Act on the Treatment of Children and Adolescents With Emotional and Behavioural Problems or Disorders in Education. Using a mixed-methods approach, we investigated how professionals in expert centres1 and social work centres, youth judges, and paedopsychiatrists, as key stakeholders responsible and competent for the professional and lawful treatment of such youth, seek and implement the most appropriate forms of assistance for them, and how they assess the implementation of the Act and the cooperation amongst them. Data were collected through four tailored questionnaires, as well as focus groups and semi-structured interviews. The research is part of a larger Slovenian study entitled Phenomenological and aetiological analysis of emotional and behavioural problems and disorders and the development of didactic approaches for specific subtypes of problems and disorders, not yet published. Based on the opinions of these experts from various fields, we identified shortcomings in the system of support, including what should be eliminated or changed, and what new forms of support and cooperation among stakeholders would be reasonable and necessary to introduce. The results of the survey are also important for understanding the competences, responsibilities, and cooperation among social work centres, expert centres, paedopsychiatrists, and youth judges.

Towards development of guidelines for harnessing implementation science for suicide prevention : an international Delphi expert consensus study

Opis: Objectives Suicide research and prevention are complex. Many practical, methodological and ethical challenges must be overcome to implement effective suicide prevention interventions. Implementation science can offer insights into what works, why and in what context. Yet, there are limited real-world examples of the application of implementation science in suicide prevention. This study aimed to identify approaches to employ principles of implementation science to tackle important challenges in suicide prevention. Methods A questionnaire about promoting implementation science for suicide prevention was developed through thematic analysis of stakeholder narratives. Statements were categorised into six domains: research priorities, practical considerations, approach to intervention design and delivery, lived experience engagement, dissemination and the way forward. The questionnaire (n=52 statements—round 1; n=44 statements—round 2; n=9 statements—round 3) was administered electronically to a panel (n=62—round 1, n=48—round 2; n=45—round 3) of international experts (suicide researchers, leaders, project team members, lived experience advocates). Statements were rated on a Likert scale based on an understanding of importance and priority of each item. Statements endorsed by at least 85% of the panel would be included in the final guidelines. Results Eighty-two of the 90 statements were endorsed. Recommendations included broadening research inquiries to understand overall programme impact; accounting for resources in the translation of evidence into practice; embedding implementation science in intervention delivery and design; meaningfully engaging lived experience; considering channels for dissemination of implementation-related findings and focusing on next steps needed to routinely harness the strengths of implementation science in suicide prevention research, practice and training. Conclusion An interdisciplinary panel of suicide prevention experts reached a consensus on optimal strategies for using implementation science to enhance the effectiveness of policies and programmes aimed at reducing suicide.
