Machine learning identifies distinct movement control impairment clusters in patients with chronic neck pain

Opis: atients with chronic neck pain experience various impairments, with reduced movement control suggested as a significant contributing factor. The heterogeneity of this patient population and suboptimal rehabilitation outcomes suggests the existence of latent subgroup characteristics. The aim of this study was to identify distinct groups among patients with neck pain based on the movement control test and pain intensity and to provide information on cluster-specific impairments. 135 patients with idiopathic neck pain performed a movement control test (the Butterfly test) at three difficulty levels and were assessed for pain intensity, providing 13 different parameters (classifiers). Louvain, hierarchical and k-means clustering methods were applied and the number of clusters determined by observing the symmetry and size of silhouette scores. Further, different machine learning algorithms were applied to develop and evaluate a classification framework (based on AUC, classification accuracy, sensitivity, and specificity) and to provide information on individual cluster characteristics using the Shapley Additive Explanations. The k-means and deep learning neural network methods provided the most efficient clustering and classification performance extracting 4 meaningful clusters. Patients between groups differed in the amount of impairment, with cluster 2 and 1 representing the most severe impairments and with clusters 3 and 4 the least severe impairments. Additionally, specific motor control impairments were observed in individual clusters suggesting distinct neck movement control adaptations. Identifying subgroups of patients with neck pain and their specific characteristics based on the results of the Butterfly test may inform future development of targeted rehabilitation strategies.

Eccentric quasi-isometric exercise produces greater impulse with less pain than isokinetic heavy–slow resistance exercise in ankle plantar flexors : quasi-randomized controlled trial

Opis: Recently, there has been growing interest in optimizing exercise protocols in sports training and rehabilitation, with particular attention to eccentric quasi-isometric (EQI) contractions, which involve maintaining joint position until isometric failure and then resisting the subsequent eccentric phase. Evidence directly comparing EQI with other contraction modes remains scarce. This quasi-randomized controlled trial examined the short-term effects of EQI versus isokinetic heavy–slow resistance (IHSR) exercises on ankle plantar flexors, focusing on pain, range of motion (RoM), and strength performance. Thirty-two physically active participants were allocated to EQI (n = 16) or IHSR (n = 16) groups and assessed at baseline, immediately post-exercise, and 24 and 48 h later. Both groups performed three exercise sets with 3 min breaks. The protocols were designed to approximate matched loading, based on preliminary testing. Nevertheless, the EQI group achieved a significantly greater total impulse (p = 0.028), a shorter time under tension (p = 0.001), and lower effort scores (p < 0.001). Group × time analysis revealed less decline in maximal voluntary isometric contraction torque (p = 0.002; η2 = 0.16), as well as lower general (p < 0.001; η2 = 0.32) and activity-related pain (p < 0.001; η2 = 0.32) in the EQI group, with no significant differences in dorsiflexion RoM (p = 0.893). In conclusion, EQI produced a higher torque impulse while inducing less fatigue and post-exercise pain than IHSR, suggesting it may be a more efficient loading strategy for the ankle plantar flexors. The results contribute to the understanding of contraction-specific efficiency, and may inform the design of future training and rehabilitation protocols targeting the ankle plantar flexors.

Psychosocial factors in the development of low back pain among professional drivers

Opis: Background and purpose: Professional drivers as a group are exposed to high risk of developing low back pain due to ergonomic factors and work conditions. The purpose of the study was to examine to what extent the low back pain occurs among Slovene professional drivers as a result of the development of various psychosocial factors. Methodology: The study involved 275 respondents (professional bus drivers, car/van drivers, international truck/ lorry drivers, and ambulance car drivers). Hypotheses were tested using multivariate statistical method (regression analysis) and analysis of variance. Data were collected by structured questionnaire comprised of three parts: socio-demographic data, basic psychosocial factors causing low back pain, and incidence, duration and severity of low back pain as a result of psychosocial risk factors, was implemented. Results: The results of quantitative survey suggest that low back pain is mostly caused by lifting and carrying heavy loads, inadequate working conditions, poor physical fitness, regular nights out, shift work, and stress. Only the impact of gender on low back pain distress among professional drivers was confirmed, predominantly among bus drivers and lorry drivers on international routes. Low back pain occurrence was less common, albeit not statistically significant, among professional drivers of vans and passenger cars. Conclusion: Our study suggests that psychosocial factors are also important cause for the development of low back pain among professional drivers and can limit the quality of their social and professional lives.
